Pathogeny and types of obesity in traditional Chinese medicine and western medicine
The causes and pathogenesis of obesity are complex. According to literature and clinical practice, its causes mainly are dampness, phlegm, aquae, stasis (fat stasis). There are natural endowment factors and factors of acquired diet habits for causing obesity. Based on the syndrome differentiation analysis of viscera, obesity is a syndrome of root deficiency and branch excess mainly caused by vital qi deficiency. The root of the disease is spleen and stomach dysfunction, yang qi deficiency, involving liver and kidney dysfunction, which can lead to turbid phlegm, water-damp and blood stasis due to stagnation of qi. The disease has different manifestation because of the difference in gender, age, diet, emotions. The following is the common types of the disease:


1, Class of damp phlegm due to splenic asthenia: the symptoms are poor appetite, plump body, sense of suppression in the chest and choke with resentment, difficulty in breathing and lack of force. Someone may suffer from edema of lower limbs or amenorrhea. It is very common in middle-aged and elderly people and can be treated by tonifying spleen, dehumidification and resolving phlegm.


2, Class of heat of the stomach leading to stagnation of the intestine: the symptoms are excessive intake of food, polyorexia, obese and robust body, flushing complexion, constipation. It is very common in teenagers and postpartum women. It can be treated by clearing stomach and smoothing fu-organ, cooling blood and tonifying intestines.


3, Class of qi depression to blood stasis: the symptoms are dysphoria and irritability, swelling pain in chest and rib, menstrual disorder, slightly dry bowels, insomnia and dreamful sleep. It can be treated by smoothing liver and regulating qi, removing stasis and eliminating fat.


4, Class of spleen yang and kidney yang deficiency: the symptoms are adiposity, coxcombical complexion, dizziness, acid and softness of waist and knee, abdominal distension, loose stool, impotence and cold sensation of the genitalia. Its common complications are high blood pressure, coronary heart disease, diabetes. It can be treated by warming kidney and invigorating yang, tonifying spleen and eliminate fat.


Obesity can be divided into simple obesity and secondary obesity as a whole in western medicine. Simple obesity is caused by non-disease factors, and it also can be categorized into two types: systematic obesity and overfeeding obesity.


Systematic obesity namely is the obesity of parents, which is caused by the increase of systematic adipocyte due to heritage. This has something to do with the excessiveness of nutrition. Patients with this kind of disease have a slowish and inefficient metabolic process. Anabolism speed is faster than catabolism in their body. Overfeeding obesity is also called acquired obesity, which is caused by conscious and unconscious excessive intake of food in adulthood, especially the intake of greasy food. And overfeed will make intake of heat more than the body needs for growth and exercises, then the surplus heat will become fat, lead to hypertrophy of adipocyte and increase the amount of adipocyte. Finally, overfeeding obesity will emerge due to the abundant accumulation of fat.


Secondary obesity is caused by diseases. Commonly, it can be divided into:
1, Thalamic obesity;
2, Endocrine obesity;
3, Hereditary obesity.
Besides the above reasons, obesity is also related to everyone’s living environment, diet habits, age, gender, amount of activity etc. At the same time, obesity also can be caused by mental stimulation and using glucocorticoids, etc.
