Syndrome of wind and heat attacking the exterior
Main symptoms and signs: Red and swollen,scorching and painful external auditory canal, or with fever and aversion to cold, diffuse swelling of the external
auditory canal or a bit of purulent secretion found during the examination, thin and white fur, floating and rapid pulse.
Therapeutic methods: Dispelling wind and clearing away heat, relieving swelling to check pain.
Prescription and drugs: The prescription used is the modified Yinqiao Powder (Powder of Lonicera and Forsythia )  composed  of  10 g  of Jinyinhua  ( Flos Lonieerae), 10 g of Lianqiao ( FructusForsythiae ), 30 g of Lugen ( Rhizoma Phragmitis ), 6 g of Bohe (Herba Menthae) (to be decocted later), 10 g of Jingjie (Herba Schizonepetae), 6 g of Jiegeng (Radix Platycodi), 10 g of Pugongying (Herba Taraxaci) and 3 g of Gancao (Radix Glycyrrhizae).
Modification: In the case of severe pain, add 10 g of Xiakucao ( Spica Prunllae) and 10 g of Huangqin (Radix Scutellariae); in the case of exudate, add 10 g of Kushen (Radix Sophorae Flavescentis ) .

Differentiating syndrome to decide treatment
Clinically, the disease mainly manifests as the syndromes of wind and heat attacking the exterior, fire and heat of the liver and gallbladder as well as hyperactive fire due to yin deficiency. At the acute stage, treatment mainly concentrates on clearing away heat and purging fire from the liver and gallbladder; at the primary stage, dispelling wind is accompanied; at the chronic stage, treatment focuses on nourishing yin and clearing away heat.

Syndrome of fire and heat of the liver and gallbladder
Main symptoms and signs: Diffuse red swelling and severer pain of the external auditory canal, or with pyorrhea, feverish body, dysphoria, thirst with inclination to drink, dry stool, red tongue with yellowish fur, taut and rapid pulse.
Therapeutic methods: Clearing away heat from the liver and gallbladder, purging fire and relieving swelling.
Prescription and drugs: The prescription used is the modified Longdan Xiegan Decoction (Decoction of Gentiana for Purging the Liver Fire) composed of 3 g of
Longdancao (Radix Gentianae), 10 g of Huangqin (Radix Scutellariae ), 10 g of Shanzhizi ( Fructus Gardeniae), 10 g of Zexie (Rhizoma Alismatis), 15 g of Shengdihuang ( Radix Rehmanniae ), 10 g of Danggui (Radix Angelicae Sinensis ), 10g of Xiakucao ( Spica Prunellae) and 3 g of Gancao (Radix Glycyrrhizae).
Modification: In the case of profuse pus, add 10 g of Chuanshanjia (Squama Manitis) and 10 g of Zaojiaoci (Spina Gleditsiae); in the case of severer thirst, add 10 g of Nanshashen ( Radix Adenophorae ) and 10 g of Maimendong(Radix Oph iopogon is ).

Syndrome of hyperactive fire due to yin deficiency
Main symptoms and signs: Itching and uncomfortable external auditory canal with a bit of purulent secretion, rough and thickened skin of the external auditory canal, or with the crust and squame, red tongue with thin fur, thready and rapid pulse.
Therapeutic methods: Nourishing yin and moistening dryness, clearing away heat and relieving swelling.
Prescription and drugs: The prescription used is the modified Zhibai Dihuang Pill (Pill of Anemarrhena, Phellodendron and Rehmannia) composed of 10 g of Zhimu (Rhizoma Anemarrhenae), 10 g of Huangbai ( Cortex Phellodendri ),  10g of Shengdihuang (Radix Rehmanniae), 6 g of Mudanpi ( Cortex Moutan Radicis ), 10 g of Shanyurou (Fructus Corni), 10 g of Fuling (Poria), 10 g of Zexie (RhizomaAlismatis), 10 g of Shanyao ( Rhizoma Dioscoreae ), 10 g of Jinyinhua ( Flos Lonicerae) and 10g of Xiakucao (Spica Prunellae).
Modification: In the case of severe itching of the external auditory canal, add 10 g of Juhua (Flos Chrysanthemi) and 3 g of Chantui (Periostracum Cicadae ) ;
in the case of skin rhagades and bleeding, add 10 g of Qiancao ( Radix Rubiae ) and 10 g of Zicao ( Radix Arnebiae seu Lithospermi ).

Other therapeutic methods

External therapeutic methods
Local medication is also important for the treatment of inflammation of the external auditory canal in addition to oral administration. The general local treatment of acute inflammation of the external auditory canal mainly includes complete disposal of pus, crust, ear wax, epidermal desquamation, etc. inside the external auditory canal. This can make the affected part clean and the drainage unobstructed.
In the case of severe otic swelling and pain, apply Qingfu Powder around the region of the external ear, once a day.
In the case of exudate inside the external auditory canal, insufflate Phellodendron Powder or Natural Indigo Powder into the affected part after washing it clean with 3 % hydrogen dioxide solution, once a day. In the case of stricture of the external auditory canal, do not insufflate the powder into it. Otherwise, it is difficult to be washed. In this case, use 30 % Phellodendron Ear Drops. Drip the drug into the ear.
In the case of local itch, use Goldthread Paste with additional ingredients [ including Kufan (Alumen Exsiccaturn), Qingfen ( Calamelas ), Bingpian ( Borneolum ),etc. ]. Apply it on the affected part with a thin cotton stick.
In the case of swelling of the external auditory canal with profuse exudate, insufflate a bit of Phellodendron Powder into the external auditory canal, once a day. Each time before the insufflation, wash the affected part clean with 3 % hydrogen dioxide solution. Or the patient may drip 20 % Phellodendron Ear Drops into the ear by himself once every three to four hours. Or plug a small gauze stripe dipped in the traditional Chinese drug Goldthread and Safflower Oils into the external auditory canal, once a day.


Simple and proved recipes
Decoct 30 g of Xiakucao (Spica Prunellae) for oral administration. It is applicable to syndromes of wind and heat attacking the exterior as well as excessive fire of the liver and gallbladder.
Decoct 20 g of Juhua (Flos Chrysanthemi) and 20 g of Banzhilian(Herba Scutellariae Barbatae) for oral administration, one dose a day. It is applicable to syndrome of wind and heat attacking the exterior and syndrome of excessive fire of the liver and gallbladder.
